any singnal to midi note convertor patches out there?

hey, me again… today’s conundrum…
I’m trying to use instruments to control synthesizers / effects. Primarily a guitar, but sometimes samples.
So I’m trying to make somthing that can convert it’s pitch to a midi note, but I’m not at all sure how to do this. I’m guessing that it’ll have to go through an fft to analyse the frequencies… but I’m to sure how to do this… or how to extract the midi data from it. If someone could please help me out, I’d be very grateful.
