I use the Expert Sleepers ES-8 as it works with audio signals rather than midi. Far more flexible in my opinion.

I have an ES8 (and ES3 and ES6 to max out the number of ports) and use it to output CV to my modular, and as input of audio to process in Max, and then route back to the modular. Works great and is easy to use. As to MIDI I have two Intellijel muMIDI interfaces to handle all the MIDI stuff (MIDI to CV). For me I prefer to think in terms of either MIDI or CV at the interface. You would have to convert your MIDI to the appropriate CV (usually 1 v/oct) on the Max side before you output it via the ES8 but there are other signals you need. With MIDI to CV I also have Gate / Trigger (note on / note off) synced to the note as well as velocity and CC info as CV. It all depends on what you are trying to do....

simply put....build a sequencer in Max to trigger note on/off in modular system
