I'd like to make a Max for Live MIDI Device that can receive MIDI notes from the channel it's loaded into, AND also MIDI notes from another MIDI channel.

In Ableton, with some inserts like Corpus you can select a MIDI ext/aux "MIDI sidechain" input. Also plugins like Kontakt, VocalSynth, and Gatekeeper receive notes from other input MIDI channels by routing to the plugin within the MIDI channel itself.

I think MIDI "merge" like this could be done in an M4L Instrument device type because they can take MIDI Input on multiple MIDI channels automatically, and can also have auxiliary MIDI Out (via the link Double_Ug presented) if I'm not mistaken. But it's not as nicely "inline" as a MIDI device would be, clearly.

As mentioned, an M4L instrument can receive MIDI on multiple MIDI channels from other MIDI tracks (multi-timbral instrument). Since MIDI input of the track where the device resides is by default on MIDI channel 1 you need to send additional MIDI on channel 2. Within the instrument the inputs must then be separated by MIDI channel.
